BY CONTINUING TO COMPLETE THE ONLINE QUIZ, CLICKING THE “CONTINUE” BUTTON OR ANY OTHER SIMILAR BUTTON, OR OTHERWISE INTERACTING WITH THE QUIZ, YOU CONFIRM THAT:

1. GENERAL PROVISIONS
These Terms of Use (hereinafter — the “Terms”) constitute a public offer and govern the relationship between:
GLK APPS LLC (hereinafter — the “Company”, “we”, “us”, “Sequoia”) and an individual (hereinafter — the “User”, “you”),
in relation to:
- the online quiz available at https://glkapps.com/;
- provision of contact details;
- purchase of a subscription for access to the Sequoia mobile application.
The agreement shall be deemed concluded from the moment the User performs the actions specified in the preamble of these Terms.
The User confirms that they have the legal capacity and have reached the age sufficient to enter into a contract in accordance with applicable law.
2. SUBJECT OF THE AGREEMENT
The Company provides the User with the opportunity to:
- complete the online quiz;
- provide contact details (including an email address);
- receive personalized quiz results (if applicable);
- purchase a subscription for access to the Sequoia mobile application;
- obtain access to the application after successful payment.
These Terms govern the above-mentioned actions exclusively.
3. QUIZ COMPLETION
The User:
- voluntarily answers the quiz questions;
- confirms the accuracy of the provided data;
- understands that the quiz results are informational and for general guidance only.
The Company may use the User’s responses for:
- generating results;
- personalizing offers;
- analytics and service improvement.
4. PERSONAL DATA
When using the quiz, the User provides personal data, including email.
The User agrees that:
- the data is used to provide access to the service;
- the Company may send the User service and informational messages, as well as marketing and promotional messages;
- the data is processed in accordance with the Privacy Policy.

5. SUBSCRIPTION AND PAYMENT
General Provisions
After completing the quiz, the User may select a plan and purchase a subscription for access to the Sequoia mobile application.
Access is granted after successful payment.
Current prices are available in Appendix 1 to these Terms.
Payment Methods
The User may pay for the subscription by bank card via the Internet.
After selecting a subscription plan and clicking the “Continue” button, the User will be redirected to a secure payment page of the bePaid processing system.
If the card supports 3-D Secure technology, the system will offer to complete a standard cardholder verification procedure on the issuing bank’s page. After payment, a confirmation will be sent to the email address provided during payment. We recommend saving payment details. Payments are accepted via the following bank cards: Visa, Visa Electron, MasterCard, Maestro, Belkart.
Payments are processed via the bePaid electronic payment system (https://bepaid.by/kak-oplatit). The bePaid payment page complies with PCI DSS Level 1 security standards. All confidential data is stored in encrypted form and is resistant to unauthorized access. Access to authorization pages is carried out using a secure data transmission protocol (SSL/TLS).
Refunds are made to the card used for the original payment. The refund processing time ranges from 1 to 30 days from the date of refund. Refund conditions are described in the “Refunds” subsection of these Terms.
Automatic Renewal
Unless otherwise specified, the subscription is recurring and automatically renews for the next period.
Subscription Cancellation
The User may cancel the subscription at any time before its automatic renewal.
Cancellation may be carried out by sending a request to customer support at: support@sequoia.health.
Cancellation must be completed no later than twenty-four (24) hours before the start of the next billing period. In this case, no charge will be made for the next period. If the subscription is cancelled during an already paid period, no refund will be issued, and access to paid functionality will remain available until the end of the paid period.
Refunds
In case of payment via acquiring, refunds are possible only in the event of a technical error (including double charge) or failure to provide the service due to the fault of Sequoia. Refunds are made to the same payment card used for the transaction.
In case of erroneous payment or dissatisfaction with the quality of the service, the User may notify Sequoia within five (5) calendar days from the date of payment by sending a request to support@sequoia.health and attaching proof of payment. Requests are reviewed individually, and the decision on refunds is made at Sequoia’s discretion.
6. ACCESS TO THE MOBILE APPLICATION
After payment, the User receives access to the Sequoia mobile application.
The User understands and agrees that:
- access may be granted automatically;
- additional registration in the application may not be required;
- login to the application may be performed using the data provided during payment.
7. APPLICATION TERMS OF USE
The User expressly agrees that:
- the purchase of a subscription constitutes acceptance of the Terms of Use of the Sequoia mobile application;
- such terms apply to the use of the application in full.

If the User does not agree with the Application Terms, they must not purchase the subscription.
8. ELECTRONIC COMMUNICATIONS
The User agrees to receive:
- service notifications;
- messages related to access and subscription;
- other messages related to the use of the service.
Messages may be sent via email.
9. INTELLECTUAL PROPERTY
All rights to the online quiz, its content, design, software, and other materials provided within the service (hereinafter - the “Content”), including trademarks, logos, and other identifiers, belong to the Company or are used on lawful grounds.
The Content is protected by applicable intellectual property laws.
Subject to compliance with these Terms, the User is granted a limited, non-exclusive, non-transferable, revocable right to use the Content solely for personal non-commercial purposes.
The User is prohibited, without prior written consent of the Company, from:
- copying, distributing, reproducing, or otherwise using the Content;
- using the Content for commercial purposes;
- creating derivative works based on the Content.
All rights not expressly granted to the User are reserved by the Company.
Any violation of this section may result in termination of access to the service and other measures provided by law.
10. PROHIBITED ACTIONS
The User agrees to use the quiz and related services solely for lawful purposes and in accordance with these Terms.
The User is prohibited from:
- using the service in violation of applicable laws;
- providing false, inaccurate, or misleading data;
- interfering with the operation of the service, disrupting its functioning, or attempting to gain unauthorized access to its elements;
- using automated means (including bots, scripts, parsers, and other technologies) to access the service or collect data;
- copying, reproducing, distributing, or otherwise using elements of the service without the Company’s permission;
- taking actions aimed at circumventing technical restrictions of the service.
The Company may restrict or terminate the User’s access to the service in case of violation of this section.
11. LIMITATION OF LIABILITY
THE SERVICES ARE PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS. YOU AGREE THAT YOU USE THE SERVICES AT YOUR OWN RISK. TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, WE DISCLAIM ALL WARRANTIES, EXPRESS OR IMPLIED, IN CONNECTION WITH THE SERVICES AND THEIR USE, INCLUDING, WITHOUT LIMITATION, IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. WE MAKE NO WARRANTIES OR REPRESENTATIONS REGARDING THE ACCURACY OR COMPLETENESS OF THE CONTENT OF THE SERVICES OR THE CONTENT OF ANY WEBSITES OR MOBILE APPLICATIONS LINKED TO THE SERVICES.
The Company does not guarantee uninterrupted operation, absence of errors, or compliance with the User’s expectations.
To the maximum extent permitted by applicable law, the Company’s liability is limited to the amount paid by the User.
12. CHANGES TO THE TERMS
The Company may amend these Terms at any time.
The current version of the Terms is published on the relevant web page and enters into force from the moment of publication, unless otherwise specified.
The User is responsible for independently reviewing the Terms for updates.
Continued use of the quiz, as well as purchasing or using the subscription after the changes come into force, constitutes acceptance of the updated Terms.
13. GOVERNING LAW AND DISPUTES
These Terms are governed by the laws of the Republic of Belarus.
In the event of any dispute, disagreement, or claim arising from these Terms, the Parties shall attempt to resolve such dispute through negotiations.
The User may submit a request to the Company via email: support@sequoia.health.
The Company shall review the request within a reasonable time.
If the dispute cannot be resolved through negotiations, it shall be submitted to the court at the location of the Company in accordance with the laws of the Republic of Belarus.

15. APPENDIX 1
Subscription Pricing
1. The following subscription plans are available:
- Monthly - from $4.99/month;
- Annual - from $19.99/year.
2. Payment may be made in Belarusian rubles at the exchange rate set by the relevant payment system at the time of payment.
3. The indicated prices are indicative (“from”) and may vary depending on:
- the user’s region;
- payment currency;
- applicable promotions, discounts, or taxes.
4. The final subscription price is displayed to the User before payment confirmation.
5. The Company reserves the right to change subscription prices. Changes take effect from the moment of publication.
